Prostate Cancer Case Studies

prostate cancer study

Patient Identification: JM
Age: 77 Years Old
PSA: 15.5 ng/ml; Rising
DRE: Non-diagnostic
Ultrasound: Scattered calification
Color Flow Doppler: Areas of hypervascularity in the Left Apico-Mid Prostate, left Mid Prostate and Right Mid Base
MRI (3.0 Tesla): Decreased Signal Intensity in Left Perpheral Zone, associated with a 4.5 mm deep Apico-Mid Prostate
Spectroscopy: Fingerprint of Disease Identified @ the Left PZ associated with a Creatine + Choline / Citrate ratio of 1.01 consistent with suspicion for Prostate Cancer
Dynamic Perfusion: Hyperemic Flow in the ROI
Biopsy: Negative in 2001 with a PSA of 7.15 ng/ml (grayscale US), negative in 2002 with a PSA of 11.5 ng/ml (Color Flow Doppler)
Diagnostic Options: Targeted Biopsy - Confirms Cancer; Gleason Score:7 (3+4)
Treatment Options:
• Brachytherapy, Cryosurgery
• High Intensity Focused Ultrasound
• Focal Therapy with any of above options

Color Flow Doppler: Area of Hypervascularity in the Right Mid Prostate Associated with Calcification
MRI (3.0 Tesla): Decreased Signal Intensity in the Right Peripheral Zone at the Base; Organ Confined
Spectroscopy: Finger Print of Cancer Identified @ the Right Peripheral Zone/Central Zone Associated with a Creatine + Choline / Citrate Ratio of 2.92 consistent with Prostate Cancer
Dynamic Perfusion: Rapid Uptake and Rapid Washout consistent with Prostate Cancer
Biopsy: Gleason Score 6 (3+3) Identified @ the Right Hemi-Prostate
Treatment Options:
• Radical Prostatectomy • Brachytherapy • Cryosurgery (Option - Focal Therapy)
• High Intensity Focused Ultrasound (Option - Focal Therapy)
• CyberKnife Radiation (Option - Focal Therapy)

prostate cancer study

Patient Identification: KW
Age: 74 Years Old
PSA: 17.0 ng/ml
DRE: Mild Atypia @ the left Lateral Edge; Non-Diagnostic for Prostate Cancer
Ultrasound: Hypoechoic Lesion @ the Apex in the Left Peripheral Zone (PZ)
Color Flow Doppler: Areas of Hypervascularity at the Left Apex, Left Base and Right Apico-Mid Prostate
MRI (3.0 Tesla): Decreased Signal Intensity @ the Left Apex Extending to the Mid Prostate; Subtle T2 Weighted Signal Decrease @ the Right Peripheral Zone; Evidence that the Cancer Process is Extending Laterally in the NVB
Spectroscopy: Finger Print of Cancer Identified @ the Left Mid Peripheral Zone Associated with a Creatine + Choline/Citrate Ratio of 2.13 Consistent with Prostate Cancer
Biopsy: Gleason Score 7 (4 +3) @ the Left Apex and Left Apico-Mid Prostate (PZ)
Treatment Options:
• CryoSurgery with a Wide Treatment Margin Laterally on the Left Side
• High Intensity Focused Ultrasound (HIFU) with a Wide Thermal Ablation on the Left
• Chronic Disease Management (Active Surveillance)
• CyberKnife Radiation
• Brachytherapy with High Dose Radiation (HDR)
• Instensity Modulated Radiation Therapy (IMRT)
